چکیده

Postoperative hypoparathyroidism (POH) is the most common complication after bilateral thyroid surgery and it characterized by low serum calcium levels with inappropriately parathyroid hormone (PTH) level. Several risk factors have been described, including technical issues (inadvertent gland excision or devascularization) patient- disease-related (sex, type of disease). Surgical strategies to minimize POH implemented, careful preservation blood supply. The spectrum severity symptoms depend on hypocalcemia; concern neuromuscular irritability may range from tingling in fingertips circumoral region paresthesias upper lower extremities, bronchospasm, laryngospasm tetany. measurement postoperative PTH following thyroidectomy critical detect promptly manage avoid symptomatic hypocalcemia. In general, a level < 15 pg/mL indicates an increased for POH. Symptomatic hypocalcemia generally well controlled oral vitamin D supplementation intravenous supplementation. Long-term consequences hypoparathyroidism, nephrolithiasis, nephrocalcinosis, basal ganglia calcification, ectopic soft tissue cataracts, defects bone metabolism, should be prevented adequate management.

Impact of postoperative magnesium levels on early hypocalcemia and permanent hypoparathyroidism after thyroidectomy.

BACKGROUND Postoperative hypocalcemia is a common complication of thyroidectomy. Magnesium is known to modulate serum calcium levels and hypomagnesemia may impede correction of hypocalcemia. The purpose of this study was to investigate whether hypomagnesemia after thyroidectomy has any impact on early hypocalcemia and/or permanent hypoparathyroidism. METHODS We conducted a retrospective revie...

Postoperative Hypoparathyroidism and the Viability of the Parathyroid Glands During Thyroidectomy

OBJECTIVES To prevent hypoparathyroidism after thyroidectomy, preservation of the parathyroid glands and their vascularity are essential. The aim of this study was to determine the association between postoperative parathyroid function and the viability of the parathyroid glands during thyroidectomy. METHODS We prospectively analyzed 111 patients who underwent total thyroidectomy and in whom ...

Normocalcaemic Hypoparathyroidism

Objective There are no consistent data on the prevalence and bone status of normocalcaemic hypoparathyroidism (NHYPO) as defined by normal adjusted calcium and low PTH level. Our aim was to determine the prevalence and the metabolic bone profile of NHYPO in older women, assessing its evolution over time. The second objective was to evaluate the prevalence of other calcium metabolic disorders. D...

Postoperative calcium levels as a diagnostic measure for hypoparathyroidism after total thyroidectomy.

OBJECTIVE The aim of the present study was to identify a fast, efficient and low-cost method to diagnose hypoparathyroidism after total thyroidectomy. MATERIALS AND METHODS One hundred and forty medical records, which contained patients' clinical and laboratory data, were retrospectively analyzed. Patient parathyroid hormone values, which were obtained immediately following operation, were co...

Hypoparathyroidism unmasked by alendronate.

The case of an elderly woman is reported in whom alendronate, given for osteoporosis, led to severe hypocalcaemia a few days after starting the drug treatment. This was caused by the unmasking of previously unrecognised hypoparathyroidism.
